This tool does not diagnose sepsis. It helps identify when infection symptoms plus signs of worsening condition may require urgent evaluation. If the result indicates higher risk, the tool can trigger an alert workflow and start a structured clinical evaluation.

How the Portal Flow Works

The patient check collects a small set of signals: possible infection plus possible deterioration. The output is a tier that routes to the safest next step.

Tier 1: possible emergency risk, immediate ED or call 911.

Tier 2: urgent evaluation today, nurse triage or same day visit.

Tier 3: monitor plus clear return precautions.

Implementation note: In production, the "Send to Care Team" action should create a message or task in the EHR, then a clinician confirms or rules out sepsis using objective measures. This MVP stores assessments in a local registry and lets a care team update status, treatment, and outcomes.
